We provide essential support to find and maintain safe and stable housing for individuals facing housing insecurity.
The Housing Stabilization Services are aimed at helping people with disabilities and the elderly find and maintain affordable housing. These services are to support individuals’ transition into housing, increase long-term stability in housing in the community, and avoid future periods of homelessness or institutionalization. Housing Stabilization Services encompass the following services:
- Housing Consultation is the initial stage where individual and housing specialists meet and develop a focused person-centered plan.
- Housing Transition is when one works with a housing specialist to search for an apartment, townhome, or house of the individual’s choice.
- Housing Sustaining is the stage where one continues to work with a housing specialist to maintain housing. The housing specialist will stay a support resource to help the individual navigate housing needs such as paying bills, finding resources for housing, and non-housing related needs.
